Please fill out this form if you are requesting a certificate of congratulations for a life milestone.

Certificates of congratulations are available for Strathroy-Caradoc residents celebrating the following milestone birthdays or anniversaries:

• 80th birthday or more;
• 50th anniversary or more.

If you would like the Municipality to provide a commemorative certificate, please complete the "Milestone Celebration" form below.

Please allow up to 4 weeks for your certificate to be prepared.

The personal information on this form is collected under the authority of the Municipal Act S.O. 2001, c.25. This information will be used to assist with processing the request for a certificate.
